We partner with referring organizations such as social workers, case managers, support planners, and care coordinators to provide needed care in the home. For more than 25 years, Luxe Healthcare has provided high-quality services to people with disabilities and older adults. We are the largest provider of services in Minnesota. Our services help people stay in their homes and lead fuller lives.

245D Waivered Services
Luxe Healthcare is a Basic 245D licensed provider of Individualized Home Supports without Training; Individual Community Living Supports; Respite; Homemaker; and Night Supervision Services. In each service, you select a trusted, qualified worker suited for your needs. Your assigned 245D Luxe Healthcare Service Coordinator will work with you and your county case manager to set up and administer your Basic 245D Waivered Services, ensuring that all workers are properly trained.
Luxe Healthcare also administers Caregiver Living Expenses, which provide reimbursement of certain expenses, such as rent and food, incurred by an unrelated caregiver who is living in the same household as an individual receiving waivered services and providing approved services to that individual. Luxe Healthcare’s Basic 245D Waivered Services include:
- Individualized Home Supports without Training: Direct supervision, cueing, guidance, instruction, assistance with activities of daily living, or assistance with coordinating and attending community living activities.
- Individual Community Living Supports: Services and support for older adults who need reminders, cues, intermittent/moderate supervision, or physical assistance to remain in their own homes.
- Respite (In Home or Out of Home): Short-term relief for the primary caregiver.
- Homemaker Services: Cleaning, laundry, meal preparation, shopping for food and supplies.
- Night Supervision: Overnight assistance and monitoring from an awake overnight staff.
